A safety anchor installed incorrectly 5+ years ago, with the mechanical fasteners exposed to the weather, and less than 1/4 of the required fasteners, on a 4 story building. And hey, they even left a rope and harness in the attic for the next tradesman to hook up to it. Not only is the owner of the building in a terrible liable position if someone connects to this anchor, but that person may be putting their life at risk.
